Job Description:

A company is looking for a Care Coordinator - Contact Agent to handle member support through inbound and outbound calls.

Key Responsibilities

Handle inbound and outbound calls to support members and complete intake screening questionnaires
Conduct audio/video initial program assessments and cold calls to enhance member engagement
Assist members with scheduling appointments and collaborate with clinicians and team leads

Required Qualifications

2+ years of healthcare/clinical experience
Call agent experience is preferred
Basic understanding of EHR/EMR applications
Proficiency in Microsoft Office
Must live in the United States
