Schedule: Monday-Friday, 8:30 AM - 5:00 PM (1-hour lunch)
Contract: 1-2 months with potential to hire

We're seeking two Community Health Nurses (RNs) to provide
high-quality, skilled nursing care and clinical assessments to PACE
members and their families. This Bronx-based role focuses on
serving a vulnerable population with compassion and
professionalism.
What You'll Do:
- Conduct skilled nursing visits and health assessments
- Deliver person-centered care using the full nursing process
- Educate members and families on health management
- Maintain documentation in accordance with regulations and
policies
- Collaborate with the care team to support holistic patient
needs
What We're Looking For:
- NYS Registered Nurse license
- Prior experience with community health, home care, or PACE
preferred
- Strong assessment, communication, and organizational skills
- Commitment to culturally sensitive and compassionate care
