Job Summary: Middletown Park Rehabilitation & Healthcare is seeking a skilled Physical Therapist to join our team. As a Physical Therapist, you will be responsible for planning, organizing, and implementing physical therapy services to help residents achieve their highest level of physical, mental, and social well-being.Duties and Responsibilities:

• Assist in planning, developing, and implementing physical therapy services in accordance with federal, state, and local standards, guidelines, and regulations.
• Develop and implement programs, goals, objectives, policies, and procedures to help residents maintain their highest level of physical, mental, and social well-being.
• Interpret and instruct physical therapy policies and procedures to personnel, residents, and family members as necessary.
• Participate in discharge planning, development, and implementation of resident care plans and resident assessments.
• Supervise Physical Therapy Assistants as outlined by the State Physical Therapy Practice Act.
• Maintain treatment records, resident files, and progress notes as required.Requirements:
  • Current license and registration to practice as a Physical Therapist in the State of NY.
  • Current malpractice insurance and license in good standing.
