Job Description:

We are seeking a skilled Registered Nurse to provide high-quality patient care in the comfort of their own homes in the Wyomissing, PA area. As a Home Health Nurse with Omni Personal Care Services, you will have the opportunity to work with a variety of patients and develop meaningful relationships with them and their families.Key Responsibilities:

• Provide personalized patient care in accordance with individualized plans of care and/or service plans.
• Conduct thorough clinical nursing assessments to ensure patient eligibility and medical necessity for services.
• Develop and revise plans of care and/or service plans in collaboration with other healthcare professionals.
• Ensure patient safety and well-being by adhering to infection control standards and federal and/or state program requirements.Requirements:
  • Current RN licensure in the state of practice.
  • Minimum of one year experience as an RN preferred.
