Job Description:
A company is looking for a Behavioral Health Care Coordinator to
provide case management services for families of medically complex
children.
Key Responsibilities
Monitor high-risk pediatric patients and adjust care plans as
needed
Provide case management and support for mental health crises and
family conflicts
Conduct telehealth visits and participate in interdisciplinary team
consultations
Required Qualifications, Training, and Education
Master's degree in social work
Preferred license/certification (LMSW) in Texas/Florida
Minimum 3-5 years of post-graduate experience in health care social
work/case management in behavioral health
Experience working with pediatric populations and family
systems
Bilingual in Spanish required
